What the price includes
The cash column is the drug plus that pharmacy’s dispensing fee, with no insurance and no telehealth fee. The savings-card column applies myZepbound Savings and Support Program on top where one publishes a basis for a figure; it generally cannot be combined with public plans such as ODB, NIHB or BC PharmaCare.

Why does the same pen cost more at one chain than another?

Every pharmacy sets its own markup on the drug and its own dispensing fee on top. In our data Costco Pharmacy is consistently lowest for Zepbound, and the gap to the priciest chain we track is $73 a month on the starting dose alone. Nothing about the medication differs.

Does the province I live in change the price?

Yes, by a few percent. Provincial rules on pharmacist fees and which chains operate differ, so Alberta is currently the cheapest province we track for Zepbound and Quebec the most expensive, a $47 monthly difference. Two Western-Canada-only chains also drop out of the comparison in central and Atlantic provinces.

Should I buy a higher strength and split it?

We publish prices, not dosing advice, and pen strengths are not interchangeable. The per-dose column exists so you can see how the cost curve moves as you titrate, not as a suggestion to redose. Any change to how you take a GLP-1 is a conversation with your prescriber.
